And you're sure that the worksheet is not protected?

What version of excel are you using? In xl2k and below, you can't insert
columns in a range that is filtered (without removing that filter). I'm not
sure if the option is greyed out, though.

Does this happen on all your workbooks--or just one?
Does it happen in all the worksheets in that workbook or just one?

If you start excel in safe mode:
close excel
windows start button|run
excel /safe

does it work ok?

How about tools|Customize|toolbars tab
select worksheet menu bar
click reset

Did that help?

And you're positive that alt-i doesn't swap between two different options???

item highlighted.

If you hit alt-i a few times, you may be able to see the highlighted options
toggle on and off.

If there are two items that have the same accelerator key, the excel won't know
which one you want. (If you hit enter with one highlighted, you may be able to
see where it is, too.)
