Check in ToolsOptionsOtherAdvanced OptionsCOM Add-Ins and Add-In Manger.
If not there, you'll need to clean it out of the registry. Look in the keys
HKEY_CURRENT_USER\Software\Microsoft\Office\Outloo k\Addins and
HKEY_CURRENT_USER\Software\Microsoft\Office\11.0\O utlook\Addins
